(WFLA) — State inspectors temporarily closed one Tampa Bay eatery from Oct. 24 to 28, 2016. We’ve listed the restaurant that was closed below, along with details about what inspectors say they found.

Villa Pizza located at 517 Brandon Town Center in Brandon

Oct. 28, 2016: Restaurant temporarily closed with 11 violations

Oct. 29, 2016: Restaurant reopened with 1 violation

The following Tampa Bay restaurants received the most violations during inspections conducted from Oct. 17 to Oct. 21, 2016.

Lin Garden located at 238 Highway 41 S. in Inverness, 33 violations

  • 6 live roaches found in dish machine area.
  • 5 dead roaches by dish machine.
  • 2 live flies on the cook’s line in the kitchen.

Elsa’s Mexican Restaurant located at 18450 U.S. Hwy 41 N. in Lutz, 30 violations

  • Self-service salad bar/buffet lacking adequate sneeze guards or other proper protection from contamination.
  • Plastic container of red sauce stored on floor in kitchen.
  • Old food stuck to clean dishware/utensils.

Trang Viet Cuisine located at 1524 E. Fowler Ave. in Tampa, 30 violations

  • Stop Sale Order issued for raw beef in freezer that employee tried to separate using a rusty screw driver.
  • Employee with no hair restraint while engaging in food preparation.
  • Soil residue in sugar container.

New York New York Pizza and Bar located at 1512 E. 7th Ave. in Tampa, 29 violations

  • Shredded cheese stored in cardboard box.
  • Employee with no hair restraint while engaging in food preparation.
  • Raw animal food stored over ready-to-eat food: raw ground beef over pepperoni in walk-in cooler.

Carmel Café and Wine Bar located at 3601 W. Swann Ave. in Tampa, 29 violations

  • Old labels stuck to food containers after cleaning.
  • Employee eating in a food preparation or other restricted area.
  • Bowl or other container with no handle used to dispense food.

Hao Wah Chinese Restaurant located at 1713 S. Dale Mabry, Tampa, 29 violations

  • Shelf under preparation table soiled with food debris.
  • Food stored on floor.
  • Accumulation of black/green mold-like substance inside the ice bin.

Bernini located at 1702 E. 7th Ave. in Tampa, 28 violations

  • Dead roaches on premises: 1 under handwash sink in bar.
  • Working containers of food removed from original container not identified by common name.
  • Cooked seafood not reaching a minimum internal temperature of 145 degrees Fahrenheit for 15 seconds: salmon 120-165°f.

Gandy Sandbar and Grill located at 10056 Gandy Blvd. N. in St. Petersburg, 28 violations

  • Dead roaches on premises: 4 dead near dish machine.
  • Accumulation of debris on drainboards.
  • Single-service articles improperly stored on floor.
