The following Tampa Bay restaurants received the most violations during inspections conducted from Oct. 17  to Oct. 21, 2016.

Jerk Hut Jamaican Grille, 926 E. Fowler Ave., Tampa, 49 violations

  • Dead roaches on premises: 1 dead under prep table, 1 dead by dish machine cleaned at time of inspection.
  • Fly sticky tape hanging over food/food preparation area/food-contact equipment.
  • Employee handled soiled dishes or utensils and then handled clean dishes or utensils without washing hands.

Village Inn Restaurant, 4401 N. Lecanto Highway, Beverly Hills, 34 violations

  • Employee eating in a food preparation or other restricted area.
  • Soil residue in food storage containers.
  • Employee handled soiled dishes or utensils and then handled clean dishes or utensils without washing hands.

Vallartas Mexican Restaurant, 13731 N. Dale Mabry, Tampa, 33 violations

  • Raw animal food stored over cooked food: raw chicken over cooked shrimp.
  • Raw animal food stored over ready-to-eat food: shrimp over rice.
  • No soap provided at handwash sink.

Paisano’s Pizza n’ Pasta, 6000 4th St. N., St. Petersburg, 33 violations

Oct. 19, 2016: Restaurant temporarily closed with 33 violations

  • Dead roaches on premises. 4 dead in dish area. 2 small and 2 large.
  • Stop Sale Order issued for dented/rusted can of tomato sauce.
  • Stop Sale issued due to food not being in a wholesome, sound condition.
  • Ready-to-eat, potentially hazardous pasta salad marked with a date that exceeds 7 days after opening/preparation.
  • Roach activity present as evidenced by live roaches found.
  • 18 live roaches, small and large, found in dish area.
  • One large, live roach near mixer.
  • One small, live roach at food server’s station.

Oct. 19, 2016: Restaurant reopened with 16 violations

Taco Bus, 1001 E. Hillsborough Ave., Tampa, 31 violations

  • Live, small flying insects in kitchen, food preparation area, or food storage area: gnats throughout kitchen.
  • Roach excrement and/or droppings present.
  • Dead roaches on premises. 1 dead in bathroom

Taste of China, 13720 Little Rd., Hudson, 31 violations

  • Rodent activity present as evidenced by rodent droppings found: approximately 20 behind freezer.
  • Unnecessary persons in the food preparation, food storage, or warewashing area: children eating at prep table.
  • Food placed in soiled container/equipment: cooked wontons and chicken in cooler at front of kitchen.

Acropolis Greek Taverna, 1833 E. 7th Ave., Tampa, 28 violations

  • Roach activity present as evidenced by live roaches found: 1 under reach-in coolers on cook’s line, 6 behind ice machine, 1 on wall, 15 under reach-in cooler and shelving next to reach-in cooler, 2 under dishwashing machine, 1 under cook’s line, 1 by gyro broiler table.
  • Roach excrement and/or droppings on wall by server tray racks.
  • Toxic substance/chemical stored by or with food: cleaner by vinegar.

 #1 Wok, 36115 E. Lake Rd., Palm Harbor, 28 violations

  • Employee with no hair restraint while engaging in food preparation.
  • Stored food not covered in walk-in cooler: cut vegetables and sauces.
  • Stop Sale Order issued for deeply dented cans present on shelf in dry storage area.
