Rodents, roaches close 5 restaurants, 1 food truck in Tampa Bay area

State inspectors found evidence of rodents or roaches and temporarily closed six Tampa Bay eateries, including one food truck, from October 12 to 16, 2015. We’ve listed the restaurants that were closed below, along with details about what inspectors say they found.

Corner Grille located at 5009 Gall Blvd. in Zephyrhills

Oct. 12, 2015: Restaurant temporarily closed with 13 violations Rodent activity present as evidenced by rodent droppings found. Observed 15 dry rodent droppings behind some single service boxes at the drive-thru window. Observed 10 dry rodent droppings under a table by the back door. Roach activity present as evidenced by live roaches found. Observed 1 live roach inside an unused, standing, reach-in freezer by the drive-thru window. Observed 2 live roaches in a dry storage room off of the cook’s line. Observed 20 live roaches underneath and inside of the tires of a make table on the cook’s line.

Dead roaches on premises.Observed 4 dead roaches under a 3-compartment sink. Observed 3 dead roaches inside the gasket of a make table by the checkout window. D’s Flavor Station located at 2150 25th St. N. in St. Petersburg

Oct 12, 2015: Restaurant temporarily closed with 9 violations Mobile food dispensing vehicle license number was not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. Roach activity was present as evidenced by live roaches found. 17 live roaches found under hand sink on the mobile food vehicle. Raw animal food was not properly separated from ready-to-eat food. Raw shrimp, raw beef, raw chicken and hot dogs were all in same Igloo-brand cooler. Items were not properly separated from each other. Inspector advised operator that all raw proteins have to be separated based on cook temperature and ready to eat items should be totally separated from raw products so there is no cross contamination.

A container of oil was stored on the floor. An employee failed to wash their hands before changing gloves and/or putting on their gloves to work with food. The manager lacked proof of food manager certification. Raw/undercooked animal food was offered but establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Oct 13, 2015: Restaurant reopened with one violation

Ice Cream Fantasia located at 12327 University Mall Court in Tampa

Oct 12, 2015: Restaurant temporarily closed with 18 violations Dead roaches were found on the premises. Two dead roaches were found on a shelf next to the ice cream chest freezer at a front counter area. Two dead roaches were found in a mobile cabinet near the soda machine. Three dead roaches were found in a door jam to a restroom door. One dead roach was found in an upright reach-in cooler in the back kitchen. Roach activity present as evidenced by live roaches found. Four live roaches were found under a soda machine cabinet area. One live roach was found in a mobile cabinet next to an ice cream freezer at the front counter area. One live roach was in a wheel to a cabinet near an ice shaver machine at the front counter area. Five live roaches were in a box in the restroom of the kitchen. One live roach was found in paper towels in the employee restroom. Two live roaches were found in a gasket to reach-in cooler next to the ice machine in the back kitchen. Oct 13, 2015: Restaurant reopened with 9 violations

Barb’s Family Dining, 5017 Gall Blvd., Zephryhills

Oct 14, 2015: Restaurant temporarily closed with 24 violations Stop Sale Order issued due to adulteration of food product. Stop Sale Order issued for Peanut butter jar due to live roach activity contaminating product. Roach activity present as evidenced by live roaches found. Observed 2 live roaches on a peanut butter jar on a storage shelf. Observed 5 live roaches at the hand wash sink on the cook’s line. Observed 3 live roaches in the tire of a make table on the cook’s line. Observed 20 live roaches under the dishwashing machine. Observed 3 live roaches near some posters by the swinging kitchen door. Observed 10 live roaches behind a poster over a 3-compartment sink. Observed 1 live roach on the hot water heater. Observed 30 live roaches in a shelving bracket above a make table.

Dead roaches found on premises. Observed 5 dead roaches at a hand wash sink on the cook’s line. Observed 2 dead roaches on top of a make table on the cook’s lline. Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing their hands. Oct 16, 2015: Restaurant reopened with 6 violations

Big City Grill Co. located at 528 Oakfield Dr. in Brandon

Oct 15, 2015: Restaurant temporarily closed with 34 violations Dead roaches were found on the premises, approximately 50 were found throughout the kitchen.  1 live roach was found on a wall near a reach-in freezer. 1 live roach was found behind a refrigerator. 16 live roaches were found on a wall and on the floor behind a reach-in cooler and make station that were not in use. 1 live roach was found in a side make station that was not in use. A Stop Sale Order was issued for potentially hazardous food due to temperature abuse, a gyro cone was used yesterday and was placed in a cooler last night, then was placed on a vertical broiler today. A container of medicine was improperly stored over a prep table with to go containers. A shelf under a preparation table was soiled with food debris. Oct 16, 2015: Restaurant reopened with 10 violations

Salem’s Gyro and Subs located at 4004 E. Hillsborough Ave. in Tampa

Oct 15, 2015: Restaurant temporarily closed with 27 violations Roach activity present as evidenced by live roaches found. 2 live roaches were found on the floor on the cook’s line under a table. 5 live roaches were found in a door frame in the kitchen. 5 live roaches were found under fryers on the cook’s line. 3 live roaches were found under some dry storage racks in the back prep room. 1 live roach was found near door to the kitchen. Dead roaches were found on the premises. 1 dead roach was on the floor near the entrance to a kitchen. 1 dead roach was found in front of a reach-in cooler on the front line. 2 dead roaches were found under the cook’s line. Live, small flying insects were found near a dry storage area. Working containers of food were removed from their original container but were not identified by a common name. Oct 16, 2015: Restaurant reopened with 9 violations

Inspectors from Florida’s Department of Business and Professional Regulation conducted the inspections. The department cited violations of Florida’s sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration’s Food Code.

While most establishments correct all violations in a timely manner (often during the inspection), the division’s procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary, according to the Florida DBPR website.
